ce signe à ce moment constitue une preuve insuffisante en
faveur d'une NAV. Cependant, si le dôme du talus reste
anormalement dense, 4 à 6 mois après le traumatisme, cela
signe une ischémie (NAV) osseuse.
Là où le CT scan est la technique de choix pour l'évaluation
de la fracture en phase aiguë et pour l'évaluation postopéra
toire de l'alignement après la réduction (Figure 5), l'image
rie par résonance magnétique (IRM) est la meilleure tech
nique pour visualiser l'étendue de la NAV.
D'autres complications sont l'ostéomyélite en cas de
fractures ouvertes, l'arthrose posttraumatique, un retard
de calcification osseuse et éventuellement une pseud-
arthrose.
concluSion
Les fractures et fracturesluxations du talus sont très rares,
mais elles doivent être soigneusement évaluées au moyen
de l'imagerie. Un traitement rapide et correct est néces
saire pour éviter les complications sévères et les séquelles.
