summary:
The Younger Onset Dementia program aims to
improve quality of life for people with younger
onset dementia, their carer and families. The
program will provide person centred support
to eligible people and their carer through the
provision of basic community care services and
supports.
The program aims to assist people and carers
plan for their future. The program trials a person
centred approach in community care. The focus
of the program is on giving choice and control
to the person and their carer by identifying their
basic community support needs - assisting
them coordinate and link into services that
will meet their current and future needs. The
program also aims to utilise a prevention and
early intervention approach by anticipating and
planning for future needs and transitional points
in a person's life.
A person centred approach is a way of
supporting and working with people and their
carer that puts the person and their carer at the
centre of planning, funding, support and service
arrangements. For this program, the person
centred approach is designed to give the person
and their carer choice about the support they
want, how and when it is received, and who
provides it. Maintaining social connections and
supporting family members are key objectives.
A personal plan will be developed for the
person and their primary carer. The plan will
coordinate access to a range of supports and
services based on the choices, aspirations
and needs of the person and their carer. The
person and their carer will be involved as
much as possible in planning, and can lead the
planning process if they choose to.
Packages will be available for both the person
and their carer and it is anticipated the person
and carer will be able to choose how they
will manage their package through one of the
following options:
Option 1 ­ Direct Payment
Funds are transferred directly to the person
and their carer who undertakes the fund
holding and support coordination roles.
Option 2 ­ Financial intermediary
Fund holding functions are performed by a non-
government service provider on behalf on the
person and their carer but support coordination
is undertaken primarily by the person with a
disability and/or their family and carers.
Option 3 ­ service intermediary
Funding holding and support coordination
functions are performed by one or more non-
government service providers on behalf of the
person. In this model, the person develops a
funded support plan which is implemented by
the service intermediary under their direction.
ADHC has engaged Alzheimer's Australia NSW
to undertake an evaluation of the pilot program.
The evaluation will be assessing the program's
process and impact in relation to meeting
a person and carer's expectations and the
effectiveness the program has on their quality
of life.
This pilot will run for three years commencing
in early 2013.
