Care Plus provides adult health and social care services to people across North East Lincolnshire to help improve
people's health and wellbeing and to enrich people's lives.
The HOPE Specialist Service, which is a partnership between the Care Plus Social Enterprises Falls and Respiratory
Services, aims to provide a patient centred one stop shop for people with chronic respiratory diseases, such as Chronic
The Health and Wellbeing Collaboratives aim to encourage people to improve their health, well-being and overall
lifestyle, enabling them to determine how they wish to live an active life and to support them to do so.
The collaboratives consist of the Early Presentation of Cancer Symptoms Programme, the Older Peoples Health and
Wellbeing Programme, Coronary Heart Disease and Skin Health Programmes.
Coronary Heart Disease Collaborative
The Coronary Heart Disease (CHD) collaborative work across North East Lincolnshire to raise awareness of risk factors
relating to coronary heart disease.
The collaborative particularly focus on trying to encourage people who have not already had blood pressure checks to
have them by o ering them in community settings. Hypertension (high blood pressure) puts a strain on the heart and
can lead to coronary heart disease. Heart disease often does not display any symptoms, but with early detection,
individuals have been successfully and easily treated.
The team will be o ering diabetes risk assessments starting in Autumn 2012. Anyone wishing to join the team to

Early Presentation of Cancer Symptoms Collaborative
The Early Presentation of Cancer Symptoms Collaborative is an exciting programme whose aim is to help people to
recognise symptoms and go for help when needed. Early detection saves lives. Survival rates from most cancers are
getting better and better. The programme raises awareness of the symptoms of bowel, prostate, cervical and more
recently lung cancer. We also raise awareness of the bowel and cervical screening programmes.
