Single Point of
Access
One telephone call to 01472 256256 or
email (A3TeamPublic@nhs.net) is the main
route for the general public to access
information, advice and guidance for adult
social care and urgent and non-urgent
community health and social care services.
This includes;
Adult social care
Intermediate tier
Occupational therapy
Physical disability services
Learning disability services
Sensory impairment team
Maintenance and review of current social
care packages
Safeguarding Adults Information & Advice
Community Care Finance Information &
Advice.
A person can also contact the single point of
access on behalf of someone they care for
or are concerned about.
This does not include access to GPs or
district nursing teams.
All calls are received by an integrated advice
o cer who will carry out a rst level of
screening and assessment. Individuals who
require further assessment are diverted to
the multi-disciplinary duty triage team where
your needs will be responded to
appropriately.
Single Point of Access is a 24/7 365 days
a year service.
