Diabetes Service
The service primarily provides training and education to primary care health providers, and to patients with Diabetes via
The service also provides practical advice and assistance in the care and treatment of adults with diabetes for GP prac-
tices, care homes and other community care providers. The service works with local commissioners and other partners to
develop provision of diabetes services in the community.
The Care Plus community infection control team o ers support, advice and education to all North East Lincolnshire
community health and social care workers on the control and prevention of infections. This includes GPs, District Nursing
Support and advice is available for patients from the team who can be contacted on weekdays via:
