Continuing Healthcare
Continuing care means care provided over an extended period of time to a person aged 18 or over to meet physical or
mental health needs which have arisen as the result of disability, accident or illness.
"NHS Continuing Healthcare" means a package of continuing care arranged and funded solely by the NHS.

An individual who needs "continuing care" may require services from NHS bodies and/or from Local Authorities (LAs).

On 1st October 2009 the Department of Health published the Revised National framework for NHS Continuing Health
Care. This guidance sets out the principles and process of the National Framework for NHS Continuing Healthcare and
and principles of care planning and dispute resolution relevant to that process.
What is Funded Nursing Care?
Healthcare then your nursing fees will be paid by the NHS. The payment is made directly to a care home registered to
provide nursing care and funds the services provided by registered nurses employed by the homes. Services provided on
a regular basis by a registered nurse are likely to involve:
your care you can seek advice from the North East Lincolnshire Continuing Health Care Service.
Your nurse and/or social worker or a professional identi ed by the Continuing Healthcare Service will rstly complete a
continuing care checklist to identify if a full assessment of eligibility is required. You will be involved in the completion of
this assessment and provided with a copy of the nal document together with information about what to do if you
disagree with the outcome.
