Community and Specialist Nursing Services
Within North East Lincolnshire, Care Plus Group has a number of Community and Specialist Nursing Services supporting
local people with their physical health needs within their own homes. Our Community and Specialist Nurses work in
partnership with local GPs, delivering care and support to those people living in the North East Lincolnshire area who
require on-going nursing intervention.
Community Nursing
Community nursing is provided to housebound patients in other words, those patients only able to leave home by
ambulance or if there is a nursing reason why a home visit is more appropriate. As the patient's condition improves,
they may be required to visit a practice nurse or a specialist clinic run by the community nursing team.
Community Nurses are based within several of the GP practices within the North East Lincolnshire area. They work
closely with the GP's in identifying and supporting adults with physical health problems within their own homes.

The Community Nursing teams have a number of sta working in them including, Health Care Assistants, Community
injections to end of life care.
We have a dedicated team of Community and Specialist Nurses and as Care Plus Group we aim to provide a high quality
of care to all our patients and their carers during what can be quite a di cult time due to poor physical health.
Continence Service
a ecting self-esteem, lifestyle and employment. Some people become virtual recluses as a result of incontinence.
The Continence service for Care Plus Group recognises the devastating impact that incontinence can have and our aim
is to provide a comprehensive assessment and treatment plan to relieve this distress. Whilst our aim is to improve or
cure incontinence, in cases of intractable incontinence products may be provided.
A continence assessment involves obtaining detailed information about your condition and you will need to keep a
record of how much you drink and how much you pass urine for 3 days. All this information helps us to o er the correct
