Community Learning Disability Team
The Community Learning Disability Team provides a range of specialist professionals and services to support adults
Health and Well-being Co-ordinators, Carers' Support Workers, Speech and Language Therapists, Occupational
Therapist, Physiotherapists, Person Centred Planning Facilitators and Learning Disability Nurses.

Disability Day Opportunities
Provided by Care4all Disability Day Opportunities provides a range of activities and opportunities within a centre and
local community based setting to support service users to enjoy meaningful, purposeful lives of their choice.
Disability Services activities can be accessed for full or half days and are also available for people who wish to
"independently refer" themselves.

Café4 is open at the Queen Street Centre, however service users have the option of bringing a packed lunch to either
centre if they prefer.
All diets are catered for including diabetic, low fat and gluten free.
