Intermediate tier
The Intermediate Tier aims to support people with health and social care needs to stay in their own home rather than
being admitted to hospital or into long term care. It provides support to people when they are discharged from hospital,
helps people to remain independent and helps to identify any on-going needs.
Intermediate tier is for anyone over the age of 18, who lives in North East Lincolnshire or who is registered with a local
GP and needs short term health and or therapy care, support to regain their independence and or to rebuild con dence.
24 Hour Telephone Triage
Care Plus Group provides 24 hour telephone triage service. Local nurses, with local knowledge of all service available in
North East Lincolnshire give advice to patients wishing health advice or access to GP out of hours appointments. Instead
of waiting for a nurse to call patients back the call handlers o ers a responsive service, there is no wait for a call back.
Patients speak directly to nurses able to give health advice. The nurses have a range of experience able to give a wide
range of advice. The service also uses a clinical decision software programme, as an aid to triaging patients; this software
is evidence based and uses the latest information.
The call handling service also sits in the same building as the Rapid Response Service and A3. Communication between
The service also covers those patients registered with the North Thoresby practice.
24 hour health telephone triage is available for patients unable to access their own GP surgeries during working hours

Community Occupational Therapy
Community Occupational Therapists support people with complex and intermediate eligible needs. Such people will
have temporary but signi cant levels or enduring loss of physical function in daily living skills. Examples are people who
cannot access their accommodation to be independent in essential daily living tasks; people requiring rehabilitation
following a stroke; people at the end of life; people who are planning to return home from temporary community care
placement; people with moving and handling di culties.
Community Occupational Therapists take referrals for adults in the above categories; children who have housing access
needs to essential facilities. Self referrals are subject to a self assessment questionnaire to check eligibility. If not eligible
people are signposted to local suppliers and do not get o ered a professional assessment.
