Transforming care
The CCG Transforming care programme is changing the way health and social care support services are
delivered across North East Lincolnshire, to improve the experience of care for people who need care services, their
families and carers. The programme is developing services to ensure adults who need care and support:
Have the opportunity to ful l their potential
Have more choice, control and are helped to live the best possible quality of life
Stay healthy and recover quickly from illness
Live independently, live with or look after their family
Can participate in their community and are treated as equal citizens in society
The CCG is developing a new way of o ering adult social care services called self-directed support. This new
approach o ers people a personal budget to spend on their care, based on an assessment of their support needs.
There are seven steps, whether you are already receiving
Care Trust Plus social care support or you are eligible for
support for the rst time:
Step 1 nd out how much money
Fill in an assessment questionnaire to nd out if you are eligible for
social care and how much help is needed. Social care is o ered to
you in the form of a sum of money called a personal budget.
Step 2 develop a support plan
You develop a support plan to explain how you will spend your
personal budget, to meet your care needs.
Step 3 get your plan agreed
The Care Trust Plus has to agree your support plan, to check your
care and support goals are safe and once agreed, you will be given
your personal budget.
Step 4 organise your money
You take as much control as you want over the money in your
personal budget. Money can be paid to you to manage, or to
friends, family, or a care manager can help.
Step 5 organising support
You may decide to use your personal budget to receive services
provided by the Care Trust Plus, or to buy services from other people
or organisations.
Step 6 living life
Get on with living your life with the care and support services
identi ed in your support plan.
Step 7 seeing how it worked
The Care Trust Plus regularly reviews your support plan with you to
check your personal budget is helping to achieve your goals.
If you already receive Care Trust Plus adult
social care services, your care manager will
discuss self-directed support with you at your
next care review.

How to be involved in Transforming care
To have a say in the transformation of local
health and social care services, join the Experts
by Experience group.
The group welcomes adults with di erent
health and social care needs and backgrounds.
It meets monthly for two hours for members
to provide their views, ideas and work with
the Care Trust Plus to change services to meet
the needs of the local community.
