initial consultation with the Neurologist.
The clinics also act as a main connection point for patient referral with all the disciplines involved with their care, for
example, speech therapist, physiotherapist, specialist nurse, district nurses, etc. Any patient who has a con rmed
diagnosis can self refer to the clinics.
We also run a support group meeting on the last Wednesday of every month (from 13:30 to 15:00) and sometimes have
speakers in to talk on various topics.

Stroke Service
The stroke service works with the Enhanced Community Stroke Team and Stroke Association to enable people to
become as independent as possible following a stroke and enable them, wherever possible, to access facilities in their
local community.
There are ve local stroke clubs who are either a liated to the Stroke Association or supported by Care Plus Group.
The Community Stroke Group on a Friday at Cromwell Road Health and Well-being Centre is a collaboration between
Care Plus and community therapists. All the clients who attend have speci c goals they need to work on to become more
independent.
