Warranty:
Beaverstate Dental warranties its product to be free of defects in material and workmanship
for FIVE FULL YEARS from the date of purchase on all units. Auto-blocks and standard
foot control (126-000) are covered by a LIFETIME warranty when factory installed as part
of a complete dental system. Warranty is limited to repair or replacement by Beaverstate
Dental. Products returned to the factory and determined to be defective will be repaired
or replaced free of charge. The warranty does not cover light bulbs. Beaverstate Dental
reserves the right to void all warranties if any product is installed by other than an authorized
technician. Beaverstate's warranty does not cover damage to any surface finish, including but
not limited to, discoloration or abrasion from cleaners, disinfectants or light sources. No claim for
labor or consequential damages will be allowed. Return items must include a copy of the invoice or packing
slip as proof of purchase.
Shipping:
All prices are F.O.B. Newberg Oregon, U.S.A.
Standard shipping will be by UPS ground. Air or expedited service is available upon request.
Minimum Order Handling Fee:
All orders less than $10.00 will be charged a $10.00 handling charge.
Terms:
Net 30 days, with approved credit. A 1-1/2% interest charge may be applied monthly to past due accounts. VISA,
Mastercard and Discover Card are accepted. When using credit cards, a 3% (of invoice) transaction fee will apply.
Returned Merchandise:
Merchandise may not be returned without prior authorization. Shipping must be prepaid. A restocking fee of $5.00 or 10%,
whichever is greater, may be charged for non-warranty related merchandise returns. Call our customer service to obtain an
authorization number and additional return information. No returns on custom or special order items, including non-standard
upholstery colors, will be accepted. Any shortages in shipment must be filed with Beaverstate Dental within 5 working days of
receipt of shipment. Claims for items damaged in transit must be filed directly with the carrier by the receiver.
